By receiving our allied health services, you consent to the practices described in this policy.

We collect and maintain information that is necessary for the provision of allied health services and for administrative purposes. This may include:

  • Personal information (e.g., name, contact details, date of birth)
  • Health information (e.g., medical history, current health conditions)
  • Billing and payment information

We use and disclose your information only for the purpose of providing you with quality allied health services. This may include:

  • Sharing relevant health information with our practitioners and staff involved in your care
  • Processing billing and insurance claims
  • Coordinating care with other healthcare providers as needed
  • Writing letters and reports to your referring Doctors/specialists, to third party provider for eg. DVA, Workcover or as required by law
  • Conducting internal audits, quality improvement, and research (with your consent)

We will not disclose your information without your consent, except as required by law.

We maintain the confidentiality of your information through appropriate administrative, physical, and technical safeguards. Our staff is trained in the importance of maintaining patient confidentiality, and access to your information is restricted to authorised personnel.

In the course of providing allied health services, it may be necessary for our practitioners to have physical contact with you. This contact is solely for the purpose of assessment, treatment, or therapeutic interventions related to your health condition. Such physical contact will always be conducted in a professional and respectful manner.

Please be assured that your comfort, dignity, and privacy will be respected at all times. If you have any concerns or preferences regarding physical contact, please communicate them with our practitioners before or during your session.

To ensure your safety and the effectiveness of your treatment, you agree to promptly disclose any significant health issues or changes in your health status that may impact your treatment or potentially harm our practitioners. Open communication about your health is essential for providing you with the best possible care.

We understand that circumstances may require you to cancel or reschedule appointments. To provide the best care to all our patients, we request at least 12 hours notice for cancellations. Failure to provide timely notice or repeated cancellations may result in a cancellation fee 50% of your scheduled appointment fee.

You understand that payment for your treatment is required on the day of service unless otherwise arranged prior to your visit. 

You have the right to:

  • Access your personal and health information
  • Correct inaccurate or incomplete information
  • Request a copy of this privacy policy
  • Withdraw your consent for the use and disclosure of your information (subject to legal and contractual obligations)
